All students who are participating in the i-FlourishWell4Life programme must complete this form as soon as possible. The following information must be provided:

1. Name and surname (first and last names);
2. Student number;
3. Email address;
4. A password that will be used to access a private and secure area on the internet with individualised reports; and
5. A nickname or display name that will be used to refer to you on i-FlourishWell4Life reports (it will be used instead of your student number and name and will therefore disguise your identity on public pages).

After you have submitted your form all the information you have entered will be sent to you via email. Please keep a copy of this email!

Individualised reports regarding your participation in online exercises and activities will be compiled and will be available on a private and secure area on the internet. You will use your student number and the password you provide below to access this area. This password is not the same as your network password and may therefore be different.
