Use this form for you weekly distribution reporting.
The fields should be self-explanatory. First, the date of Friday in the week you visited the distribution point.
Second, the actual distribution point you are reporting on (you report on each point individually).
Third, level of stock left in the container, which may be either: none, few (approx. 1 - 20%), many (20 - 80%), and full (80%+).
Finally, indicate how many units (blue packets of 10 condoms) you added.
Deadline for submission: Friday (until 24:00) of given week.
