The plain X rays show an expansile lesion of the third metatarsal shaft. The ends of the bone are sparred but the rest appears as an expansion of the bone. Pressure effects are seen in the 2nd and 4th rays which have bowed away and adapted to this slow growing lesion. The magnetic resonance show a well demarcated non homogenous, expansile lesion of the third metatarsal shaft. It contains cystic inclusions in which fluid levels can be seen. An expansile cyst containing other cystic areas with (multiple) fluid levels is pathognomic of an Aneurysmal Bone Cyst. The cyst was excised en bloc
